Consent and Liability Waiver

I, the undersigned, hereby consent to participate in a hot wax session provided by Pseudo Dragon Creations. I understand and acknowledge the following:

Nature of Treatment

Hot wax involves the application of heated wax to the skin, which may cause temporary redness, sensitivity, or mild discomfort.

Risks and Precautions

I understand that hot wax may cause burns, allergic reactions, or skin irritation. I confirm that I have disclosed any skin conditions, allergies, or sensitivities I may have. I agree to inform the practitioner immediately if I experience any discomfort during the session.

Medical Conditions

I confirm that I am not currently suffering from any medical condition that may endanger the wax practitioner or myself, including but not limited to skin infections, or open wounds.

Liability Release

I release and hold harmless Pseudo Dragon Creations and its practitioners from any and all liability, claims, or damages that may arise from the hot wax application. I understand that I am requesting the application voluntarily and at my own risk.

Privacy and Confidentiality

All personal and medical information provided will be kept confidential and used solely for the purpose of providing safe and effective wax application.

Client Acknowledgment

I have read and understood the above information. I have had the opportunity to ask questions and all my questions have been answered to my satisfaction. I consent to receiving the hot wax application and agree to the terms outlined above.
